Transition is a natural process and for most of us, transition is moving from one life setting to another is complicated and challenging, as we move from adolescence to adulthood, changing a living situation, or experiencing a death of parent or caregiver are some of the most difficult experiences, though we somehow manage but for people with disabilities or special needs, it is often tough. This manual “Effective transition for students with multiple disabilities- A model” is an attempt to explain the transition along with a scale for assessment and curriculum in a simple language along with illustrations for easy understanding of professionals and parents.
